
Dr. Mowbray’s office is affiliated with numerous health insurance plans, and we “accept assignment” under Medicare. Most of our patients do not require pre-authorization from their insurance company in order to be seen by our office. However, we do see some patients insured by “HMO-Managed Care” health insurance plans. In order to be examined by Dr. Mowbray, these patients must first have an authorization number from their PCP doctor. This number authorizes Dr. Mowbray to examine them for a specific skin problem. The patient may also be required to get a new authorization number from their PCP doctor before Dr. Mowbray may proceed with any necessary investigations or skin surgery. It is the patient’s responsibility to confirm that their authorization number has been forwarded from their PCP doctor to our office prior to their appointment.
We will file your claim with your insurance company and later notify you of any charges owed. Patients with certain insurance plans will be requested to pay their “co-payment” on the day of their visit. Cosmetic and a variety of “medically unnecessary procedures” not covered by your insurance company must be paid in advance of the procedure.
We accept cash, credit cards (Visa, MasterCard, Discover and American Express) and checks (with a valid driver’s license). Discounts may be available for multiple procedures performed on the same day.
